OSHA fines two Wisconsin construction companies after Lambeau Field carpenter’s death

The Occupational Safety and Health Administration last week fined two Wisconsin contractors after a carpenter died at a construction site at Lambeau Field in June.

In June, a construction carpenter died of his injuries after being trapped at a construction site in the football field.

The carpenter worked for Mavid Construction, a subcontractor for Miron Construction. Both companies received fines on Nov. 15, according to online records.

OSHA fined Miron Construction for $18,976 and Mavid Construction for $15,626 for serious violations. Both companies have until Dec. 14 to fulfill the abatement deadline.

Online records showed controls for an electric dumbwaiter weren’t locked and tagged, leaving construction workers exposed to a pinch-point hazard.

Miron officials said in a statement that the company received a notice from OSHA regarding the incident after what happened on June 15, 2023.

“We have been working with the proper officials and will continue following their established and trusted processes as we are committed to providing safe and productive project sites for all our team members. Miron has a longstanding rapport with OSHA and we expect that to continue,” Miron officials added.

The Daily Reporter has also reached out to Mavid Construction for comment.

On June 15, first responders answered a report of a man trapped inside Lambeau Field while construction was ongoing. After an extrication, emergency crews rushed the man to the hospital.

The man was identified as Joshua Shaw, 27, according to an obituary. He died on June 17 at St. Vincent Hospital in Green Bay.
